**We are approved by Medicare. If your insurance is in-network, you only pay your co-pay. The type of visit will be decided when you book based on an evaluation. Virtual visits may become in-home visits on the same day if the provider thinks a more thorough exam is needed (extra fees may be charged). We serve patients across Florida and Georgia for Tele-Health visits.
All forms and paperwork to be completed by provider will require 2 business day turnaround and assess a fee of $50.

Weight management plan $75/per month
Our medical assisted weight management program provides a personalized approach to reaching your weight loss goals
This plan requires a 6 month commitment which will be billed monthly. These visits can not be substituted for urgent care visits.
